Diabetes Self-Management Education Program

The diabetes self-management program is designed to empower our community to take charge of their diabetes. Our goal is to address the following measurable desired outcomes:

• healthy eating
• being more active
• blood sugar monitoring
• taking medications as ordered
• problem solving
• reducing risks
• healthy coping

Please use the following protocols for a diabetes referral:

1. Contact Cathy Fierro, RN, Diabetes Educator at (509) 840-5210 or e-mail at info@sunnysidehospital.org
2. Fax the diabetes services form to (509) 837-9058 using the attached form
3. Referrals can be made thru centricity
4. The Diabetes Education department will then contact the client for an education assessment and make necessary arrangements for group or individual education
